Psychological Causes of ED – Between 10% and 20% of ED cases have a psychological cause. Because arousal starts in the brain, psychological issues can be a significant contributing factor to erectile dysfunction. Mental health conditions like depression or anxiety can negatively impact your libido, making it more difficult for you to become aroused.
This inflatable penile prosthesis has 3 major components. The 2 cylinders are placed within the corpora cavernosa, a reservoir is placed beneath the rectus muscle, and the pump is placed in the scrotum. When the pump is squeezed, fluid from the reservoir is transferred into the 2 cylinders, producing a firm erection. The deflation mechanism is also located on the pump and differs by manufacturer.
When we say it’s a barometer of men’s health, it’s a signal. It’s an indicator that things may be right or not. And so when a man develops an erectile problem– and we’re talking about something that is occurring over time. It’s not something that just occurred overnight. When it occurs overnight, it’s more often than not a psychogenic, an anxiety reaction.
The penis contains three cylinders, the two corpora cavernosa, which are on the top of the penis (see figure 1 below). These two cylinders are involved in erections. The third cylinder contains the urethra, the tube that the urine and ejaculate passes through, runs along the underside of the penis. The corpus spongiosum surrounds the urethra. Spongy tissue that has muscles, fibrous tissues, veins, and arteries within it makes up the corpora cavernosa. The inside of the corpora cavernosa is like a sponge, with potential spaces that can fill with blood and distend (known as sinusoids). A layer of tissue that is like Saran Wrap, called the tunica albuginea, surrounds the corpora. Veins located just under the tunica albuginea drain blood out of the penis.
My fiancé and I have been together for four years, and while we've had our ups and downs, we're in a good place now and looking forward to our life together. Throughout our relationship, we've made some bad financial decisions. Since I'm the one with the credit cards (his credit is awful), I'm the one that's more affected. In some cases, nocturnal penile tumescence testing is performed to find out whether the man has erections while asleep. Healthy men usually have about four or five erections throughout the night. The man applies a device to the penis called a Rigiscan before going to bed at night, and the device can determine whether he has had erections. (If a man is able to have normal erections at night, this suggests a psychological cause for his impotence.)
The key to this line is to act chill. An occasional soft dick ain’t no thang. But if you get angry, throw off the sheets, and stomp around the apartment while your partner sits in bed watching a grown man throw a temper tantrum, it becomes a thang. Throw out an apology to cover your bases, and then let it go. Once again for the guys in the back: Women understand about not always being able to get wet. And we really understand about not being able to get off every time. One in three women has trouble reaching orgasm during sex. I get that it’s not the exact same thing, but we’re empathetic. A good rule of thumb for almost all insecurities is the less of a big deal you make about something, the less others will.
Men, if you can't get an erection and are in a relationship with someone you deeply care about... Please.. Bring on the toys.. bring on the hands.... bring on the tounge.. do SOMETHING... Don't use that as an excuse not to erouse the woman you love. If shes not getting satisfied from you, she will find it somewhere else OR... she will be sad stuck in a relationship STARVING for sex and have pity on you. You can do so much without an erection. That's not the end all be all in orgasms for women. Trust me... You need to over compensate for problem. You can actually appear MORE manly by stepping up and making sure woman is satisfied. Don't let your bed be a graveyard.!!! I am pretty sure you can still have an orgasm without an erection.. if you have the right woman,... pleasure her. Do what it takes to help her orgasm... and then it's your turn. She will make sure you are pleased regardless if you are fully erected or squishy... Don't fret over this.. Seriously...!!! Don't feel like your manhood is any less... Rise up and serve and she will make you happy. Trust me!!
If surgical intervention is an acceptable option, the work-up then proceeds with a noctural penile tumescense (NPT) test, the single most valuable study to establish the diagnosis of organic impotence. The NPT test exploits the fact that males from birth to old age normally have erections during rapid eye movement (REM) sleep. Psychogenic impotence is associated with normal erections during sleep. In organic impotence there should be no evidence of erection or, if erections are present, they should be limited and poorly sustained.

The role of the endothelium in erectile function became clearer with the observation that the phosphodiesterase type 5 (PDE5) inhibitor, sildenafil, enhanced erectile function. Erection occurs with the release of nitric oxide (NO) from the vascular endothelial cells.17 The reduction in endothelial cell production of NO results in the negative impact on the smooth muscles in the corporal bodies and results in less relaxation of the smooth muscle cells with decrease in blood supply and resulting ED. A similar phenomenon is well known to impact the coronary arterial system resulting in CVD.

Diabetes is an example of an endocrine disease that can cause a person to experience impotence. Diabetes affects the body’s ability to utilize the hormone insulin. One of the side effects associated with chronic diabetes is nerve damage. This affects penis sensations. Other complications associated with diabetes are impaired blood flow and hormone levels. Both of these factors can contribute to impotence.
Psychosocial problems are important and may cause erectile dysfunction by themselves or together with other causes of erectile dysfunction, such as diabetes and heart disease. Relationships are complicated and many factors cause tensions, which can affect sexual relations. For some men, these problems can become ongoing and it can help to talk through the issue with a skilled counsellor. It is important to know that the longer erectile dysfunction is left untreated, the greater the effect on relationships. This is another reason why early treatment of erectile dysfunction is important.

PDE5 inhibitors, the primary second-line therapy, have been the mainstay of ED treatment since the release of sildenafil (Viagra) in 1998, with the subsequent development of many others, and still more in the development stage. These medications do improve erectile quality for the majority of men, and they work by enhancing blood flow in the corpora cavernosa. These medications are generally used on demand and need to be taken about an hour before sexual intimacy. Tadalafil (Cialis) is longer acting and does come in a daily preparation potentially eliminating the ‘on-demand’ need. The daily dosing of tadalafil, 2.5–5 mg\day, has also been approved by the FDA for treatment of symptoms of BPH.41 PDE5 inhibitors are contraindicated in men taking nitrates, but otherwise PDE5 inhibitors are very safe and effective. When PDE5 inhibitors are coadministered with nitrates, pronounced systemic vasodilation and severe hypotension are possible. Many patients with ED are elderly and have the same risk factors as patients with CAD, so these drug combinations are commonly considered or encountered in clinical practice.42

In other cases, men who habitually use alcohol or other drugs may experience similar results. Alcohol is a depressant to the central nervous system. This means that a little bit might be able to lighten the mood, but too much can basically shut down all communication between the brain and the penis. When this happens, no amount of will or stimulation will result in an erection. Other drugs can also affect the body’s ability to achieve an erection, including heroin and MDMA, otherwise known as ecstasy.
Vacuum devices for ED, also called pumps, offer an alternative to medication. The penis is placed inside a cylinder. A pump draws air out of the cylinder, creating a partial vacuum around the penis. This causes it to fill with blood, leading to an erection. An elastic band worn around the base of the penis maintains the erection during intercourse.
