Vascular: Anything that affects the flow of blood to the penis can result in erectile dysfunction. The main culprit tends to be atherosclerosis, the condition that narrows arteries and which can result in poor blood circulation, high blood pressure, heart disease and stroke. Atherosclerosis causes about half the cases of erectile dysfunction in men over 50. Also, the veins through which the blood leaves the penis may not be working properly, allowing the blood to leave too soon.
The motivation and expectations of the patient and his partner and education of both are critical in determining which therapy is chosen and in optimizing its outcome. If single therapy is ineffective, combining two or more forms of therapy may be useful. Penile prostheses should be placed only after patients have been carefully screened and informed. Vascular surgery should be undertaken only in the setting of clinical investigation and extensive clinical experience. With any form of therapy for erectile dysfunction, long-term followup by health professionals is required to assist the patient and his partner with adjustment to the therapeutic intervention. This is particularly true for intracavernosal injection and vacuum constriction therapies. Followup should include continued patient education and support in therapy, careful determination of reasons for cessation of therapy if this occurs, and provision of other options if earlier therapies are unsuccessful.

Conditions that may be associated with ED include diabetes, [25, 26, 27] hypertension,  , and CAD, as well as neurologic disorders, endocrinopathies, benign prostatic hyperplasia,  , sleep apnea  , COPD,  and depression (see Table 1 below). [32, 33, 34, 35] In fact, almost any disease may affect erectile function by altering the nervous, vascular, or hormonal systems. Various diseases may produce changes in the smooth muscle tissue of the corpora cavernosa or influence the patient’s psychological mood and behavior.
In other cases, men who habitually use alcohol or other drugs may experience similar results. Alcohol is a depressant to the central nervous system. This means that a little bit might be able to lighten the mood, but too much can basically shut down all communication between the brain and the penis. When this happens, no amount of will or stimulation will result in an erection. Other drugs can also affect the body’s ability to achieve an erection, including heroin and MDMA, otherwise known as ecstasy.
I'm a college guy who has only had sex a handful of times, and I've noticed a bit of a reccurring issue. As a retired police Sgt. Mid 50's I always had a few anxiety or medicine related ED' s but those were women that didn't matter and I always made sure to have a laugh and good time. Now Been with one great women 16 years. Now I'm heavy, drink more than I should, take pain med for chronic pain and watch tv. Anyone here recognize this. how about those recreational drugs I don't do. Before complaining do what I just started. Lose weight, stop reading this and shut off the tv, STOP smoking, go to the Dr. Get blood work done, talk to someone even or especially your better half, talk to and trust your Dr. Trust me, he's heard much worse. Trust that inner voice or gut, it's there for you. And mostly stop saying what about me and my winky, and take care of the other one who loves, cleans, sleeps and whatever else he/she does For u. This won't work for all but I know somehow it will help... AND YOU MIGHT BE DAMN SURPRISED!!!
Concerns that use of pornography can cause erectile dysfunction have not been substantiated in epidemiological studies according to a 2015 literature review. However, another review and case studies article maintains that use of pornography does indeed cause erectile dysfunction, and critiques the previously described literature review.
So what can you do to help things along a little? Well, chances are it may sort itself out as you settle in to your relationship. But if it keeps happening and you see a future in the relationship, you need to talk about it, or it will soon become the elephant in the bedroom. Pick a time with no distractions and where nobody can overhear you. Tell them that you care about them and that you don’t need to rush things sexually. This can take the pressure off the need to perform.
